I had the exact same thought as Mystic_Eve!! Cytotec, when used to induce labor, fits 4 of the 5 "reasons for concern" as stated by the FDA: a lack of approval for the specific indication (labor induction); the extent to which American doctors are using the drug; maternal safety risks (uterine rupture, likelihood of c-section); risks to the infant (from unnaturally long & strong uterine contractions which deprive the baby of oxygen). As for their fifth reason for "concern", women would not have to buy Domperidone online if the FDA made it available, safely & affordably, from local pharmacies. Yet another politically driven, non-evidence based decision from the FDA. I wouldn't be surprised to find out that the formula industry had stuck their nose into that conference call.

Doesn't it also smack of politics that they are actively going after the off-label use of domperidone BUT despite numerous warnings aren't actively going after hospitals that use misoprostol (cytotec) off-label of labour induction?

I guess a doctor being on time for the golf game is more important than reducing mom's and baby's lifetime risks of many cancers, diabetes, obesity, osteoporosis, heart disease...

I am an IBCLC and certified nurse midwife practicing in the U.S.

I am a frequent prescriber of domperidone as I take care of women who are providing breastmilk to their premature and high risk infants and are experiencing great stress and decreased milk supply as a result of their infant's critical condition. I am amazed at how well it works and the success women have with it. It is unfortunate that only compounding pharmacies in the U.S. can make it which makes it expensive as insurance won't cover it. We, thankfully, have a wonderful compounding pharmacy and supportive pharmacist in the area who provides it for our local moms (and he ships domperidone throughout the U.S.).

We need to initiate studies in the U.S. to evaluate the safety of the use of this drug to increase milk supply to gain FDA approval and improve its availability to women in the U.S.
